Our Support Coordination service is designed to guide you through your plan, connect you with the right services, and build your confidence so you can take control and live your best life.

What is Support Coordination?

Support Coordination helps you understand your NDIS plan, connect with the right services, and build the skills to manage your supports with confidence.
It’s a funded support under the Capacity Building budget and is designed to help you make the most out of your plan and achieve your goals.

The service helps you…

Understand your NDIS plan and what it lets you do.

Connect with NDIS providers, community services, mainstream supports and other government services.

Build your confidence and skills so you can coordinate your supports and make your own decisions.

Level 1

Support Connection helps you link with the services and supports in your NDIS plan. At this level, we assist you with understanding your plan, finding the right providers, and answering questions as they come up, without ongoing coordination or long-term planning.

Level 2

This level is for participants who need more regular support to manage their NDIS plan. We help you review your supports, identify any changes needed, and ensure your plan aligns with your goals.
We also assist you to find providers, coordinate services, and manage the ongoing parts of your supports so everything stays organised.

We’ll check in regularly, evaluate what’s working/not working, adjust plans and supports, and continue building your capacity.

Frequently Asked Questions

  •  A: No — it depends on your individual plan, goals, level of support needs and whether your plan includes that funding under Capacity Building.

  •  A: Yes. You can choose who provides your support coordination services — registered or unregistered depending on level.

  •  Plan Management deals with the financial side of your plan (paying invoices, managing budget). Support Coordination helps you understand, use and coordinate your supports and services.

  • A: If you have significant challenges or high support needs, you may have Specialist Support Coordination funded. We can help you identify if that’s your case.
